Your membership starts upon registration and initial consultation.
Your initial membership is a 12 month agreement, with a monthly membership fee of $500. This annual agreement allows our initial medical assessment, specialized fitness and nutrition plans, and quarterly wellness visits, to have the time to take shape for your health. All payments to Prime Health are through credit card, including Visa, Mastercard, American Express, and Discover, and are automatically drafted monthly through a recurring billing platform, on the 1st of the month.
You have 30 days from the day you register to change your mind to receive a full refund of your first month’s membership fee, minus a $149 registration fee, provided you have not yet come in for your initial visit with Dr. Bond.
If you have already had your initial visit with Dr. Bond and are dissatisfied with your service for any reason, if it is within 365 days of your initial registration date, you may cancel your membership for a cancellation fee of $599.

Prime Health members have unlimited email contact with the Prime Health team. However, if an email question or request is deemed by one of Prime Health’s team to be too complicated or involved for a response via email they may, at their discretion, phone you or require that the member schedule an in-office visit instead of responding via email.
No, we are not part of an insurance plan, nor do we accept insurance for services. We advise all members to maintain a separate health insurance plan of their choosing. You should also note that the Affordable Care Act includes a general requirement that all U.S. citizens purchase health insurance. A Prime Health membership does not meet this requirement.
At Prime Health we offer preventative wellness services. While we may offer many services that are similar to those provided by a primary care physician, we do not fulfill an insurance company’s requirement that you select a primary care physician in your network, as we do not participate with any insurance carriers. We also require that you have a primary care physician of record as we do not handle medical or mental health emergencies.
We are available by phone or email 24 hours a day, but if you require urgent care you should immediately call 911 or contact an emergency care provider such as a local hospital or emergency room. The costs associated with emergency care services are not covered by your Prime Health membership. This is an Age Management and Wellness Program, NOT an urgent care or emergency room.
No, Prime Health does not bill insurance companies directly for services.
The cost of any wellness services provided by Prime Health which are not included in the membership fee will be billed to you directly.
Your membership may not be shared, assigned, or transferred to anyone else, even if you are not actively using the membership during your term.
Before your first scheduled visit to Prime Health, you will be required to provide medical and health background information through our questionnaire and to sign certain releases/authorizations and other documents regarding the care to be provided. You will not be able to access our Prime Health services until this paperwork is completed.
